What to Expect & Bring

At Lincoln Family Medicine Center, we want your experience with us to be a good one. If this is your first time scheduling an appointment with us, please request that we email or mail any new patient forms so you can fill them out in advance. 

Preparing for your appointment will help you feel more comfortable when you arrive. Take the time to gather personal and medical information to bring with you, including:

Insurance Card

If you don't have insurance, we will work to provide the best affordable care.

Photo ID

A valid form of identification might be a state driver's license, military or government-issued card, or a passport. Please call us if you don't have one of these forms of ID so that we can let you know what else may be accepted.

Medical History

Bring your completed Medical History forms and information.

Bring a list of medications you are currently taking (prescription and over-the-counter drugs, supplements, and vitamins) including the dosage and frequency, or bring your medication bottles with you.

When you check in, you will be assigned to a team of physicians and other healthcare professionals that will monitor your care. You may be seen by a physician from the Lincoln Family Medicine Residency Program, a Faculty physician, a nurse, and other members of the care team as needed. They will go over your medical history with you and discuss the medical issue(s) that you are currently having. We believe that involving you in your healthcare plan is key to you staying healthy in the future.
